ΑΝΑΤΟΜΙΑ

Άρθρωση είναι ο τόπος συνάντησης δυο ή περισσοτέρων οστών. Η άρθρωση επιτρέπει την κίνηση των οστών. Τα οστά της σπονδυλικής στήλης ονομάζονται σπόνδυλοι. Οι σπόνδυλοι ενώνονται μεταξύ τους με τις ζυγοαποφυσιακές ή αποφυσιακές αρθρώσεις.

Κάθε σπόνδυλος έχει δυο ζευγάρια αποφυσιακών αρθρώσεων. Οι δύο αποφύσεις, μια σε κάθε πλευρά του σπονδύλου, που συνενώνονται με τον επάνω σπόνδυλο λέγονται άνω αρθρικές αποφύσεις και οι δύο κάτω,  ονομάζονται κάτω  αρθρικές αποφύσεις.
Βρίσκονται στο πίσω μέρος των σπονδύλων.
Οι αποφυσιακές αρθρώσεις φέρουν αρθρικό θύλακο και οι αρθρικές επιφάνειες είναι καλυμμένες με χόνδρινο ιστό ώστε να είναι λείες και να μπορούν να κινούνται χωρίς τριβές.
Οι αρθρώσεις επιτρέπουν την κάμψη, την έκταση και την στροφή της σπονδυλικής στήλης παρέχοντας σε αυτήν την απαραίτητη σταθερότητα για να εκτελεί τις προαναφερόμενες κινήσεις.
Κάθε άρθρωση νευρώνεται από δύο νευρικούς κλάδους οι οποίοι διακλαδίζονται από τα οπίσθια στελέχη των οσφυϊκών νωτιαίων νεύρων. Οι κλάδοι αυτοί πορεύονται στην αύλακα που σχηματίζεται από την συνένωση των άνω αρθρικών αποφύσεων με τις εγκάρσιες αποφύσεις.

ΠΟΝΟΣ ΤΩΝ ΑΠΟΦΥΣΙΑΚΩΝ ΑΡΘΡΩΣΕΩΝ

Οι αποφυσιακές αρθρώσεις ενέχονται ως αιτία πόνου στο 15-45% των ασθενών με χρόνια οσφυαλγία. (14-17) Η πάθηση είναι πιο συχνή στις μεγάλες ηλικίες και μπορεί να οφείλεται σε εκφύλιση, καταπόνηση και φλεγμονή. (18,19) Ευρήματα οστεοαρθρίτιδας και γενικότερα εκφύλισης των αρθρώσεων σε ακτινολογικές εξετάσεις είναι το ίδιο συχνά τόσο σε ασυμπτωματικούς όσο και σε συμπτωματικούς ασθενείς και δεν αποτελούν διαγνωστικά κριτήρια.
Οι ασθενείς με οσφυαλγία από τις αποφυσιακές αρθρώσεις παραπονούνται για πόνο κεντρικά και ίσως ελαφρά παρασπονδυλικά χαμηλά στη μέση, χωρίς σαφή εντόπιση, που πιθανόν να αντανακλά στους γλουτούς και στην οπίσθια επιφάνεια των μηρών μέχρι τα γόνατα. Όταν πάσχουν οι ανώτερες αποφυσιακές αρθρώσεις, ο πόνος μπορεί να αντανακλά στα ισχία και την πλάγια επιφάνεια των μηρών μέχρι τα γόνατα. Πολύ σπάνια ο πόνος μπορεί να επεκτείνεται και κάτω από τα γόνατα. Ο πόνος επιδεινώνεται στη όρθια στάση και το κάθισμα και βελτιώνεται με την κατάκλιση. Το πρωί παρατηρείται συνήθως πόνος και δυσκαμψία κατά την έγερση.
Κατά την φυσική εξέταση οι ασθενείς παρουσιάζουν πόνο στην πίεση παρασπονδυλικά, στην έκταση και τη στροφή της σπονδυλικής στήλης, ενώ συνήθως αναφέρουν ανακούφιση από την κάμψη της Σ.Σ. Η νευρολογική εξέταση αν δεν υφίσταται άλλη παθολογία είναι φυσιολογική.

ΔΙΑΓΝΩΣΗ

Η κλινική εξέταση και ο ακτινολογικός έλεγχος δίνουν κάποιες πληροφορίες αλλά δεν μπορούν να θέσουν τη διάγνωση του επώδυνου συνδρόμου των αποφυσιακών αρθρώσεων.
Η διάγνωση τίθεται με τον αποκλεισμό των νευρικών κλάδων που νευρώνουν τις ύποπτες αρθρώσεις, με τοπικό αναισθητικό. Εφόσον ο ίδιος ο πάσχων διαπιστώσει σημαντική, πάνω από 50% ανακούφιση από τον πόνο του, για τη χρονική διάρκεια της δράσης του τοπικού αναισθητικού και είναι σε θέση να εκτελέσει δραστηριότητες που προηγούμενα αδυνατούσε, τότε και μόνον τότε τίθεται η διάγνωση του συνδρόμου των αποφυσιακών αρθρώσεων. (54,55,57,58,59)

ΘΕΡΑΠΕΙΑ

Η συντηρητική θεραπεία περιλαμβάνει το συνδυασμό της λήψης μη στεροειδών αντιφλεγμονωδών φαρμάκων, παρακεταμόλης και φυσικοθεραπείας  ή χειροπρακτικής . Σε περίπτωση που ο πόνος επιμένει μετά από 6 εβδομάδες συντηρητικής αντιμετώπισης, η θεραπεία εκλογής είναι η απονεύρωση των αποφυσιακών αρθρώσεων με εφαρμογή ραδιοσυχνότητας μέσω βελόνας –ηλεκτροδίου στους νευρικούς κλάδους που νευρώνουν τις επώδυνες αρθρώσεις. (75-82)
Η τεχνική εφαρμόζεται με τοπική αναισθησία στο δέρμα και υπο ακτινοσκοπική καθοδήγηση. Είναι απόλυτα ασφαλής όταν εκτελείται από ειδικό επεμβατικό ιατρό πόνου και το αποτέλεσμα διαρκεί από 9-24 μήνες. Αν τα νεύρα αναγεννηθούν και ξαναεμφανιστεί πόνος, η θεραπεία μπορεί να επαναληφθεί όσες φορές χρειαστεί. Η θεραπεία είναι επιστημονικά τεκμηριωμένη και υπάρχει ισχυρή σύσταση για την εφαρμογή της από τη διεθνή επιστημονική κοινότητα.
